Parent Transition Planner
This on-line Parent Transition Planner is part of a package of support Bexley Voice have developed for parents/carers who are supporting their child with special educational needs or a disability, as they move from Year 9 into adulthood.
It is designed to support parents/carers as they prepare their young person for adulthood in the years leading up to the move from Children’s services to Adult Social Care. However, it may also be helpful to those whose children do not have social care currently as it highlights other things that need to be considered, including Health and Independent Living.
The move from childhood to adulthood can be a stressful time of change for all young people and their families. It can lead to anxiety and questioning about who they will become as adults and how they will make their way in life and achieve their aspirations. For young people with disabilities and special needs this period can be even more daunting.
Your child or young person will be working with their school to prepare for their future. This planner will help parents/carers to support them at home and to find the information they need, when they need it. The Planner should be read in conjunction with the information available on the Preparing for Adulthood section of the Bexley Local Offer:
