Please complete a Core Information for Emergency Hospital Admission for your loved one if they have specific care and support needs.
A parent of a disabled young person was very concerned about the prospect of her son being admitted alone to hospital in the future with COVID 19. The thought of not being able to accompany her son due to illness or not being allowed due to hospital protocol in a crisis situation filled her with anxiety. She decided to prepare a Core Information Emergency Hospital Admission Sheet which outlines the key information that hospital staff would need to know to support her son. This parent received feedback from 10 other parent carers of children and young people with disabilities and all felt it was a very good idea. She then sought feedback from a medical clinical who confirmed that all the most important information is included and particularly mentioned that the tips were excellent especially laminating and photographing the information. He confirmed that the information she had included would be very important to the team and facilitate appropriate care.
We all want to ensure our precious children are cared for in the right way and that this parent hopes her initiative will help others.
Not everything on the template will be applicable to your child - but it might offer a good starting point for you to develop something that works for your family.
